The minimum age requirement is 19 years old (international age), but also, your bones have to be fully mature before you can have any of the surgeries you want. You have to do a bone density test to show that your bones are fully mature to have surgery, and if you have not turned 19 yet, then you need parent's permission along with the bones density test.
